If you are paying by credit card, please fax or mail the completed form. If you are paying by check or money order, please mail the form with your check. If you prefer, you may call in your credit card number. Our mailing address and phone number are listed on the form. DO NOT send your credit card information by email.

*** DCCW = Diabetes Core Curriculum Workshop. If you have attended this workshop before, you qualify for the discounted rate only if your payment is received by the early registration deadline. Full tuition is due after the early registration deadline has passed.
** ' 3 or more' people must be registered together. Please put the names of your colleagues on your registration form and send tuition in the same envelope.
