Terms and Conditions

Course Information

Space is available on a first come, first serve basis. Registration is only available for the entire 4-day conference. Single day registrations and on-site registrations are not available. Walk-ins will not be accepted on any day. Call us two weeks prior to the start of the conference to ensure space availability.

We reserve the right to cancel a program in the event of unforeseen circumstances. Registration fees will be refunded. Pro Health Consulting is not responsible for monies registrants may have spent on non-refundable airline tickets, penalties associated with changing airline tickets or hotel deposits in the event a course is cancelled or you are unable to attend the course.

Course schedule and faculty are subject to change. Pro Health Consulting cannot be held responsible for any losses, damages or injuries attendees may experience while on-site at the conference facility.

A certificate of course completion is provided at the end of each course. If the certificate is lost or damaged and you require a duplicate copy, a $20 fee will be assessed.

Registration tuition and contact hours are based on attending all sessions during the four day conference. If absent, partial credit will be given.

The Diabetes Core Curriculum Workshop does not receive commercial support.

Payment

Payment is due prior to the first day of each course. We accept Mastercard, Visa, check, money order or purchase order for payment. Checks must be drawn on a US bank payable to Pro Health Consulting. Insufficient check funds will be assessed a $20 fee. If a purchase order is requested, we MUST receive the tuition payment before the start of the course.

Refunds

You will receive a full refund of your tuition if we receive written notification by March 26, 2012. If a refund request is received between March 26 and April 9, 2012, there will be a cancellation fee of $75 assessed. NO Refunds will be given after April 9, 2012. (A substitute may attend in your place if we are notified in advance either in writing or by phone.) Pro Health Consulting cannot be held responsible for lectures or days missed due to illness, travel delays or flight cancellations.

Hotel and Parking

A limited number of rooms at the Sheraton LaGuardia East Hotel are being held at special discount pricing. Attendees must make their own arrangements. If the Sheraton is full, there are additional hotel options listed here. Please make your hotel arrangements early as hotels in New York fill up quickly. If you have difficulty obtaining a room, please contact us.

Parking at the Sheraton is limited and is NOT guaranteed. The Sheraton is located in an urban setting with an onsite valet parking facility. On-street parking and parking lots are within walking distance of the hotel. Prices vary; parking is not guaranteed.

Consider mass transit for your travel arrangements. The #7 Subway line and the following bus lines are within 1 block of the Sheraton: #48, 20, 52, 44, 17. Entrance for the subway is on Roosevelt Ave. Bus stops are on Roosevelt and Main St.

If you are arriving by plane into LaGuardia airport, the Sheraton offers a complimentary shuttle for hotel guests. If you are arriving via JFK airport, you will need to take a taxi to the hotel or arrange alternate arrangements. Click here for a map of parking options near the Sheraton Hotel.
